Description
The incident occurred at one mile from the start of the journey, which probably was a return journey to the base. The driver was making a left turn when the trailer disconnected from the tractor mid way through a turn, sliding off the road and overturning. Some of the tubes moved within the structure of the trailer. There was not a release of hydrogen due to the accident itself.
As a safety precaution before up righting, the company engineers and the local responders decided to vent the product from the trailer tubes in a controlled manner.
The trailer was successfully up righted without further incident. The roadway was open during the entire process.

The INITIATING CAUSE was that the trailer disconnected from the tractor during manoeuvring.
The ROOT CAUSE was attributed to human error, because it was found that the driver had not used standard practices when connecting the tractor to a trailer.

Corrective Measures
The driver was fired, due to their failure to follow the correct tractor-to-trailer connection procedure.
Since the incident was due to failing to follow the procedure for connecting a tractor to a trailer, all employees were reminded of proper the connecting procedures.
