Root CAUSE analysis
The INITIATING CAUSE was an explosion caused by the manual release (cut) of hydrogen from a pipe which had not been purged.
The ROOT CAUSE could be attributed to inadequate procedures, however, also a human error or lack of safety management would be an option.
