Description
On the way home, a trailer/truck overturned due to excessive speed, causing the hydrogen bundles to fall. It was transporting two bundles with 30 cartridges each. One of the bundle valves broke, causing gas to leak and ignite. The fusible valve (thermally activated relief devices) of the bundle also activated.
The trailer was returning to the base after delivery, and the cylinders had a residual pressure of approximately of 7 bar.

Root CAUSE analysis
The INITIATING CAUSE was the loss of the load of the trailer.
The ROOT CAUSE is usually attributed in a case of road crashes to human error (too high speed). However, the design of the pallets should be able to protect the (safety) valve system, from the type of mechanical impacts expected from a road incident.
